Use the link below to the online form to register for a vaccine. Current eligibility in Connecticut: Ages 16 and up, effective April 1, 2021.

PLEASE NOTE: The Town of Bethel clinic is only able to accept ages 18 and higher at this time. Parents of 16 and 17 year olds are advised to seek the Pfizer vaccine via other clinics.

Important notes:

  • After you fill in the required information on the registration form and click “Submit,” you will receive a “thank you” message. This will be your confirmation that you have successfully registered.
  • You will be contacted by a clinic volunteer to schedule your appointment when your vaccine dose is available.
  • Please be patient. Although we are receiving vaccine deliveries weekly, supplies are limited. It could take several weeks before your appointment can be scheduled.
  • Please be advised that vaccines may also be available through the State of Connecticut system or through select pharmacies. You can find more information at the state website by *clicking here
  • The state registration system is separate from the Bethel system. You may register with both systems and take advantage of the first appointment available through either.
